Bintaro Yamaguchi was provided with the sighting by a man who had been a forestry worker in the past. In the early 2000s, he and a group of colleagues were surveying a mountain south of Mt. Sobo, which straddles Kumamoto, Miyazaki, and Oita prefectures, for forestry work.

Of the five members, the witness was walking farthest back to retrieve a surveying pole. Then, when they came to an open area, he saw a strange creature about 2 to 3 meters away. He witnessed a black snake-like creature with a worm-like texture on its body surface and a head with only a mouth. Its body was 4 cm in diameter, and its body protruded 40 cm above the ground, making it appear to be 1.5 meters long, including the part hidden in the ground. When he looked around, he found others like it, four in total.

The creature was threatening the man with a mouth full of teeth. When he prodded it with a pole, it made a motion similar to a snake attempting to bite a man. In addition, the creature made a spitting motion at the man with spit in its mouth, so he sensed danger and went to colleagues.

Witnesses testified that it resembled the Mongolian Death Worm.
